No, they know what's wrong. It's called adhesive capsulitis, or "frozen shoulder," and what happens is that the tissues around the capsule of the shoulder have thickened, resulting in pain and reduced movement. Sometimes therapy will alleviate it, but not this time, and in that case arthroscopic surgery is necessary to cut open the adhesions, followed by therapy to make sure they stay free enough to allow proper movement while the tissues heal. It's more common in diabetics, but no one's really sure exactly what causes it.
